Important information about the Trust and your policy:

BlueCross BlueShield:

Effective January 1, 2012, the Trust has contracted the services of BlueCross BlueShield. Any participant who is covered with the PPO option the Trust offers will now be covered through BlueCross BlueShield.

There will be no changes in deductibles, out-of-pocket and lifetime maximums. Most providers that were in-network with UMR and Highmark are also in-network with BlueCross BlueShield and you will find a larger option of in-network providers with BlueCross BlueShield than with the previous plans. The Trustees made the convenience of participants a high priority when deciding to change PPO options.

New cards will be issued for all PPO participants. For information regarding providers participating with BlueCross BlueShield, please contact BlueCross BlueShield at www.myhealthtoolkitfl.com or call 800-810-2583.

First Health Network for Non-Medicare Participants

Great News! Effective October 1, 2009, the Trust has contracted with First Health Network. First Health is a network of medical providers who agree to offer their services at a discount. This discount applies to services rendered on or after October 1, 2009. This service is only available to participants who are not yet eligible for Medicare and who are not currently enrolled in the Trust’s PPO option.

Please Note:

A. If you have a question regarding your pension, please contact your former employer.

B. If you are eligible for ANY part of Medicare, you are not eligible for the PPO option of coverage and you must notify the Trust immediately.
